The term Multiple Chronic Conditions (MCC) means that a person is living with two or more chronic conditions at the same time. This is the largest, fastest growing, and costliest U.S. patient population. As the number of chronic conditions increase, there is a reduction in quality of life, inability to carry out physical activities of daily living and increased risk for frequent hospitalization and earlier death.
Currently, 1 out of 4 Americans have MCCs and for persons 65 and older 3 out of 4 have MCCs.
Total costs in the U.S. for direct health care treatment for chronic health conditions totaled
$1.1 trillion
equivalent to nearly six percent of the nation's GDP.
The most expensive conditions in terms of direct health care costs are diabetes, CV and kidney disease.
